How Do I Start Therapy?
If you are interested in scheduling an evaluation for your child, call our office with your health insurance information.

After a quick phone consultation, we will call your health insurance carrier and determine your benefits. We are currently an in-network provider with most BlueCross BlueShield plans and Qualcare plans. Otherwise, you can access our services using your out-of-network benefits.

Our friendly office staff will take care of all insurance submissions for you, as well as any authorizations or referrals that you may need.

Once you have scheduled an appointment, please fill out the Medical History Form. You can either email or fax the completed form to us prior to your child's first appointment.

What do I need to bring to my child's first appointment?
• Completed Medical History Form
• Completed Sensory Profile (for occupational therapy evaluation)
• Health Insurance Card
• Form of payment: Cash, Check, MasterCard, Visa
• Socks for you and your child. Shoes are not permitted in our treatment areas.
• A blanket and a bath towel if your baby is less than 6 months old
